        This puppy is sold to the buyer for the sum of $______________. A deposit of $_____________
has been placed on the puppy to hold the puppy until it is ready to leave. The remainder of
$_____________ must be paid in full when the puppy is picked up. A fee of $100.00 is required to
hold the puppy until it is ready to go home with a new family. This deposit fee is nonrefundable.

         We, Wayne and Janice Clabaugh, warranty the buyer the following .................

1. Seller is the owner of the puppy and has the right to sell it.
2. The puppy is healthy at the time of the sale.
3. The puppy will be current on vaccinations and dewormings.
4. A shot record will be given to the buyer.
5. The puppy has no disabilities or defects to our knowledge.

         The buyer has had the opportunity to examine the puppy and has done so. The puppy is
being sold in a healthy condition, and the seller is expressly held harmless for all sales. A condition
of the sale is that the buyer agrees to have the puppy examined by a licensed vet, of their choice,
within 7 days of the sale. If the buyer's vet finds this puppy to have a birth defect, the buyer is to
notify the seller within 9 days of the sale. The buyer must provide a letter from their vet stating his
findings. At this time the breeder will take the puppy back, refunding the money for the sale price, or
a puppy of equal value will be given if one is available. If after 9 days from purchase a claim has not
been made, all rights to a refund or another puppy are forfeited. We, the breeder will not be
responsible for vet bills incurred after the purchase.

          We would love to hear from you and get an update on the puppy from time to time.
